Maryland Medical Cannabis Card Cost & Renewal

Maryland state registration has been free since July 2024. The only out-of-pocket cost is the physician certification, typically $85–$150 via telehealth. The card is valid for three years. At Maryland’s 12% recreational excise tax, a $200/month patient saves roughly $288/year in tax alone — the card pays for itself in 4–8 months.

Total Cost Breakdown

Maryland is among the most patient-friendly states on the cost side. Combined with New Jersey’s free digital card, it ranks at the top of the affordability tier.

Most telehealth platforms refund the evaluation fee if the provider does not approve a certification.

Tax Savings vs. Recreational

Maryland applies a 12% retail excise tax on adult-use cannabis (rising to 15% by 2030 under HB 556 / SB 516, 2023). Medical cardholders are exempt from that tax. Over a year of regular purchases, the tax savings often dwarf the certification cost:

Spending LevelAnnual 12% Tax (Recreational)Saved With Medical Card
$100 / month~$144~$144 saved
$200 / month~$288~$288 saved
$400 / month~$576~$576 saved
$800 / month~$1,152~$1,152 saved

Bottom line: Patients spending $100/month or more on cannabis typically save more on tax than they pay for the certification — and state registration costs nothing. Once the tax rate rises to 15% in 2030, the math gets even better.

Other Card Benefits Beyond Tax

  • Higher possession limit: 120g / 30 days (physician-set) vs 1.5 oz (42.5g) recreational.
  • 4 home-grow plants per patient vs 2 plants per household for recreational.
  • Age 18+ access vs 21+ for recreational.
  • Higher edible potency caps available to medical patients.
  • Statewide medical-only delivery.
  • Stronger legal protections under Md. Code, Health-General § 13-3301 et seq.

Renewal Process

Maryland medical cards are valid three years. Renewal process:

  • Start at least 30 days early. Avoid any gap in tax-exempt access.
  • New provider certification required. Each renewal cycle requires a fresh certification from a Maryland-registered provider. Annual provider check-ins are also recommended.
  • Same MCA portal. Renewals are processed through cannabis.maryland.gov.
  • Still free. State renewal has no fee.
  • If your card expires before renewal, you must submit a new application. During any gap you pay the 12% rec tax and lose the higher limits.

The Maryland Cannabis Administration regulates the medical cannabis patient registry. State registration has been free since July 2024.

Updating Card Information

  • Address change. Free. Update through the MCA portal.
  • Lost or damaged card replacement. Free. Most patients use the digital card so a physical-card replacement is rare.
  • Caregiver change. Update through the portal; the new caregiver must register.
  • Legal name change. Update with supporting documentation (court order, marriage certificate, etc.).

Is the Card Worth It Post-Legalization?

  • You spend $100/month or more on cannabis — the 12% tax savings cover certification cost; state registration is free.
  • You are 18, 19, or 20 — the medical card is the only legal route to dispensary access.
  • You need higher limits (120g / 30 days vs 1.5 oz for rec).
  • You want 4-plant home cultivation instead of 2.
  • You want statewide medical delivery — rec delivery not yet authorized.
  • You qualify only under the catch-all — the certification may be the only path to documented therapeutic use.
