Maryland legalized adult-use in July 2023, but a medical cannabis card still matters. The Maryland Cannabis Administration (MCA) issues digital cards free since July 2024, and cardholders are exempt from the 12% recreational excise tax, qualify at age 18 (vs 21 for rec), get higher purchase and home-cultivation limits, and keep statewide medical-delivery access.

Program Overview

Maryland’s medical-cannabis program began in December 2017 under the Natalie M. LaPrade Maryland Medical Cannabis Commission (now folded into the Maryland Cannabis Administration / MCA). At its 2022 peak, patient enrollment reached approximately 162,000; after recreational legalization in July 2023, that number declined to roughly 86,700. Patients who stayed enrolled did so for tangible reasons: tax exemption, age-18 access, higher limits, broader product access, and medical-only delivery.

Maryland is one of the most patient-friendly programs in the country on the cost side: the state registration is free, the qualifying-conditions list includes a catch-all ("any condition severe and treatment-resistant"), and telehealth certifications are widely available for $85–$150.

The Maryland Cannabis Administration administers the medical cannabis patient registry. Registration is free as of July 2024.
