Program Overview
Maryland’s medical-cannabis program began in December 2017 under the Natalie M. LaPrade Maryland Medical Cannabis Commission (now folded into the Maryland Cannabis Administration / MCA). At its 2022 peak, patient enrollment reached approximately 162,000; after recreational legalization in July 2023, that number declined to roughly 86,700. Patients who stayed enrolled did so for tangible reasons: tax exemption, age-18 access, higher limits, broader product access, and medical-only delivery.
Maryland is one of the most patient-friendly programs in the country on the cost side: the state registration is free, the qualifying-conditions list includes a catch-all ("any condition severe and treatment-resistant"), and telehealth certifications are widely available for $85–$150.

At the current 12% tax rate, a patient spending $200/month saves $24/month or $288/year in taxes alone. With telehealth certifications running $85–$150, the card pays for itself within 4–8 months — on tax savings alone.
