Before You Begin: Eligibility
To apply for a Maryland medical cannabis card you must:
- Be a Maryland resident with a valid state-issued ID or driver’s license. Out-of-state patients buy as recreational customers — see reciprocity.
- Be at least 18 years old. Minors may be registered with a parent/guardian as caregiver.
- Have a qualifying condition — named in statute or fitting the catch-all.
- Not hold a commercial driver’s license (CDL). Federal regulations prohibit CDL holders from using cannabis under any circumstances, regardless of state law.
Step 1: Confirm Maryland Residency
Maryland residency is verified using an MVA-issued driver’s license or non-driver ID. If you have recently moved, update your ID with the Maryland Motor Vehicle Administration before applying. Out-of-state IDs are rejected by the MCA portal.
Step 2: Complete a Provider Certification
A Maryland-registered provider (physician, podiatrist, dentist, nurse practitioner, certified nurse midwife, or physician assistant who has registered with the MCA) must evaluate your medical history and submit a certification electronically. You do not upload it yourself — the provider enters it directly into the MCA system.
Telehealth Is Permitted
Maryland allows full telehealth cannabis evaluations. Most patients use one of these platforms:
- Quick Med Cards — typically $85–$129
- Green Relief Health — typically $99–$149
- NuggMD — typically $99–$149
- Heally — typically $89–$150
Most telehealth visits last 10–15 minutes. If a provider does not approve a certification, most platforms refund the fee.
What to Bring to the Visit
- A list of your current medications
- Records of your qualifying diagnosis (specialist notes, imaging, lab results, VA records, etc.)
- Your Maryland ID
- For catch-all certifications: documentation that prior treatments have been ineffective
Step 3: Register on the MCA Portal
Once your provider submits your certification, you create or log in to your patient account at cannabis.maryland.gov. During registration you provide:
- A copy of your Maryland driver’s license or state ID
- Proof of Maryland residency (if requested)
- A passport-style photo
- Caregiver information (required for minors; optional for adults)
Step 4: Confirm the Card — No State Fee
Maryland state registration has been free since July 2024. Previously the state fee was $50. As of 2026 there is no MCA registration fee, no physical-card surcharge, no annual renewal fee from the state — only the physician certification cost (typically $85–$150). This makes Maryland one of the most affordable medical-cannabis programs in the country alongside New Jersey’s free digital card.
Some patient categories — veterans, seniors, hospice, SSI/SSD/Medicaid recipients — have historically received priority processing or fee waivers; see the MCA patient page for current categories.
Step 5: Download Your Card
After your application is reviewed by MCA, your digital medical cannabis card is available to download immediately from your patient portal account. The digital card is valid at all 103+ Maryland licensed dispensaries. Physical cards are typically not required. You can begin purchasing as soon as the digital card is issued.

Common Reasons for Application Denial or Delay
- Provider not registered with MCA. The certifying provider must be MCA-registered.
- Out-of-state residency. Maryland ID required.
- Photo problems. The portal requires a passport-style photo; sunglasses, hats, and side-angle photos are rejected.
- Mismatched information. The name on the certification, ID, and application must match.
- Catch-all without supporting documentation. Catch-all certifications require evidence that other treatments have been inadequate or that the condition is severe.
Special Cases
Patients Under 18
A custodial parent or legal guardian must register as the patient’s designated caregiver. The caregiver acquires, possesses, and administers cannabis on behalf of the minor. Both the minor and the caregiver register through the MCA portal.
Caregivers for Adult Patients
An adult patient may designate caregivers; caregivers must be 21+, Maryland residents, and pass a background check. Caregivers register their own MCA accounts.
Ages 18–20
The medical card is the only legal route to cannabis purchases for Marylanders aged 18 to 20. This is particularly relevant for college students at University of Maryland, Johns Hopkins, Towson, Salisbury, and other Maryland institutions who have qualifying conditions.
